VISITATION

NOTICE: INFORMATION ABOUT CORONAVIRUS - VISITATION IS NOW POSSIBLE PURSUANT TO REGULATORY GUIDANCE. VISITATION IS SUBJECT TO SUSPENSION AT ANY TIME IF THE REGULATORY CRITERIA DESCRIBED BELOW ARE NOT MET.

ON JULY 10, 2020 THE NEW YORK STATE DEPARTMENT OF HEALTH PUBLISHED GUIDANCE ALLOWING NURSING HOMES TO RESUME LIMITED VISITATION FOR FACILITIES THAT MEET CERTAIN CRITERIA STARTING JULY 15, 2020, INCLUDING NO COVID-19 POSITIVE RESIDENT OR STAFF CASES FOR 28 DAYS. ON SEPTEMBER 15, 2020 THE DEPARTMENT OF HEALTH ISSUED UPDATED GUIDANCE REVISING THE ONSET TIME FRAME TO 14 DAYS BEGINNING JULY 17, 2020. THE REVISED GUIDANCE CONDITIONS VISITATION ON VISITORS PRESENTING A VERIFIED NEGATIVE TEST RESULT TAKEN WITHIN SEVEN DAYS PRIOR TO VISITION AND REQUIRES NURSING HOMES TO REFUSE VISITATION TO INDIVIDUALS WHO FAIL TO PRESENT SUCH NEGATIVE TEST RESULT, EXHIBIT ANY COVID-19 SYMPTOMS OR DO NOT PASS SCREENING QUESTIONS. AS OF OCTOBER 8, 2020, AMSTERDAM NURSING HOME WAS ABLE TO RESUME VISITATION. VISITATION IS SUBJECT TO SUSPENSION AT ANY TIME, HOWEVER, IF AMSTERDAM NURSING HOME IS UNABLE TO SATISFY ANY OF THE CRITERIA, INCLUDING AS A RESULT OF ANY COVID-19 POSITIVE RESIDENT OR STAFF CASE.

AS OF TODAY, AMSTERDAM NURSING HOME IS CLOSED TO ALL VISITORS BASED UPON DEPARTMENT OF HEALTH REQUIREMENTS. VISITORS WILL ONLY BE ALLOWED FOR LIMITED EXCEPTIONS, INCLUDING MEDICALLY NECESSARY SERVICES AND END OF LIFE SITUATIONS. WE WILL UPDATE THIS NOTICE WITH RELEVANT CHANGES.

WE APPRECIATE YOUR ONGOING SUPPORT AND COOPERATION DURING THESE UNPRECEDENTED TIMES, AS OUR PRIORITY REMAINS PROTECTING THE HEALTH AND SAFETY OF THE ENTIRE AMSTERDAM COMMUNITY.

PLEASE CONTACT THE SOCIAL WORKER ASSIGNED TO YOUR LOVED ONE TO ARRANGE VISITATION. WE WILL KEEP FAMILY MEMBERS AND NEXT OF KIN UP INFORMED. IF YOU ARE NOT RECEIVING AUTOMATED VOICE MESSAGES FROM AMSTERDAM NURSING HOME, PLEASE CONTACT THE SOCIAL WORKER ASSIGNED TO YOUR FAMILY MEMBER TO BE ADDED TO THE DISTRIBUTION LIST. INFORMATION AVAILABLE TO FAMILY MEMBERS AND NEXT OF KIN INCLUDES COVID-19 RELATED STATISTICS REQUIRED BY EXECUTIVE ORDER 202.18.

WE CONTINUE TO TAKE PRECAUTIONARY MEASURES TO LIMIT POSSIBLE EXPOSURE AND KEEP OUR RESIDENTS AND STAFF SAFE AND HEALTHY BASED ON GUIDANCE PUBLISHED BY CENTERS FOR MEDICARE AND MEDICAID SERVICES, CENTERS FOR DISEASE CONTROL AND PREVENTION AND NEW YORK STATE DEPARTMENT OF HEALTH.

VACCINATIONS

Amsterdam Nursing Home is participating in the Pharmacy Partnership for Long-Term Care Program developed by the U.S. Centers for Disease Control and Prevention (known as the CDC) to coordinate COVID-19 vaccination. Amsterdam Nursing Home has been partnered with CVS Health. Pharmacists from CVS Health will provide on-site vaccination clinic services to residents and staff. CVS scheduled the first on-site vaccination clinic on January 7, 2021 and the second on-site vaccination clinic on January 28, 2021. A member of our social work or nursing team will reach out to you to obtain consent if your loved one lacks capacity. If a team member has not reached out to you yet or if you have any questions, please contact the social worker assigned to your loved one.
